FORMS:

Utility Billing Account Set-up Application: For anyone setting up a new account. This includes new residents, transfers, & builders needing a new meter set. This form sets up your water, sewer, and trash services account.

Fire Hydrant Meter Set-up Application

Utility Billing Account Termination Form: For anyone moving out of Westlake. A simple form that ensures you are not charged for utilities after you’ve moved out.

Water Leak and Adjustment Review Form: If you have experienced a major leak or pool fill that may affect your Winter Quarter Average, use this form to request an adjustment. Please review the eligibility requirements found on our Leak Adjustments page.

ROW Contractor Registration: All contractors working in the ROW must register online.

ROW Construction Permit: All contractors working in the ROW must secure a ROW Construction Permit.

Network Node Permit: All contractors installing network nodes in the Town must secure a Network Node Permit.

Duct Bank Access Form: For anyone accessing the duct bank manholes. Does not include the small hand holes at the property line.
